- Repeated identical directory listings are flagged after three reads, with recovery instructions to open a specific file or narrow the search. Exhausted loop recovery now ends with a completed blocker summary instead of a failed `LOOP_DETECTED` turn.
- Plan mode stays opt-in: automatic complexity hints no longer ask to enter a read-only plan flow. Explicit plan requests still prompt the user to choose.
