Fix loop guards that interrupted productive debugging or missed stagnation.

- Edit/test cycles continue when results change; cycle detection requires repeated results and error status.
- Unchanged results are caught even after a command previously changed its output.
- Compare complete trimmed tool output with SHA-256, including diagnostics beyond 4,000 characters.
- Preserve repeat-streak resets when intervening work returns new information.

Includes regression tests. Unfinished sandbox and TUI meter changes in the main working tree are excluded.
